Personal injury chiropractor in Jacksonville, FL — for attorneys and paralegals
Send your car accident and injury clients to a chiropractic office that treats on a letter of protection, returns records the same day, and provides MMI and impairment ratings. Referrals accepted from every state.
What your case gets
A treating chiropractor who makes the file easier to settle.
Meridian Integrative Wellness is a chiropractic practice with three chiropractic physicians and two offices in the Jacksonville metro. We have treated musculoskeletal injuries from car accidents, work, sports, and everyday life since 2010. Attorneys refer to us because the documentation arrives on time and reads the way an adjuster needs it to.
Letters of protection accepted
We treat on an LOP (your state may call it a medical lien, doctor's lien, or treating on assignment). Itemized, CPT-coded billing and a copy of the LOP are available on request, as Fla. Stat. §768.0427 now requires for LOP treatment.
Records returned same day
Send the HIPAA authorization and the request; records, itemized bills, and progress notes go back to your office the same business day. No chasing.
MMI and permanent impairment ratings
Our chiropractic physicians document maximum medical improvement and provide permanent impairment ratings — the documentation Florida's permanency threshold (§627.737) turns on.
Narrative reports written for adjusters
Mechanism of injury, objective findings, diagnosis, causation, treatment plan, prognosis. Written to be read by an adjuster or a jury, not just another clinician.
Same-day appointments, 14-day PIP compliance
New-client exams are available same day at both offices, so nobody misses Florida's 14-day window. If a client already missed it, tell us — the case is different, not over.
Imaging on site
Digital X-ray in office. When findings warrant it, we refer for MRI and incorporate the read into the record.
Refer a client
Send a client in under a minute.
We call the client the same business day, confirm the appointment back to your office, and send the initial exam narrative after the first visit. One contact for scheduling, status, and records.
How it works
- 1
Send the referral
Form, fax (888-502-0725), or phone. Include the LOP if the client is treating on one and a HIPAA authorization so records can flow to your office.
- 2
Same-day exam
History, orthopedic and neurological exam, digital X-ray when indicated, and a treatment plan with goals and a re-evaluation date. The 14-day PIP clock is respected.
- 3
Documentation on your timeline
Progress notes each visit. Records, itemized CPT-coded bills, and narrative reports returned same day on request. MMI and impairment rating at discharge.

Florida rules your case will run on
If the crash happened in Florida, Florida law generally governs — even if your client and your firm are somewhere else.
Informational summary for referring counsel, not legal advice. Confirm with Florida counsel.
14-day PIP rule
Fla. Stat. §627.736(1)(a): initial care must be received within 14 days of the crash or PIP medical benefits are forfeited. Chiropractic physicians are covered providers.
$10,000 / $2,500 PIP
PIP pays 80% of reasonable medical expenses up to $10,000 — but only $2,500 unless an emergency medical condition (EMC) is documented by an MD, DO, PA, or ARNP. Chiropractors cannot make the EMC determination; we tell you promptly when a client needs that evaluation.
Permanency threshold
Fla. Stat. §627.737(2): non-economic damages in a motor vehicle case require a permanent injury within a reasonable degree of medical probability (or significant scarring, significant loss of an important bodily function, or death). A documented impairment rating is where that showing usually lives.
Two-year statute of limitations
Negligence actions accruing after March 24, 2023 carry a two-year limitations period (§95.11, HB 837). Earlier accidents keep the four-year period.
Modified comparative negligence
§768.81(6): a plaintiff found more than 50% at fault recovers nothing. Objective, timely documentation of injury and mechanism matters more than it used to.
LOP disclosure
§768.0427 (2023): for care under a letter of protection, the claimant must disclose the LOP, itemized CPT/HCPCS-coded bills, and whether the attorney referred the client to the provider. We document so that disclosure is painless.

Referring from another state
Your client is in Jacksonville. Your firm doesn't have to be.
We accept clients who were hurt here while traveling I-95 or I-10, stationed at NAS Jacksonville or Naval Station Mayport, attending school, wintering in Florida, or who moved after the crash. We also treat Florida residents whose counsel is in another state.
Whatever your state calls the arrangement — letter of protection, medical lien, doctor's lien, attorney lien letter, treating on assignment, med-pay, no-fault / PIP, UM/UIM — tell us on the form and we will confirm how it maps to Florida. We send records to any state, respond to records subpoenas, and keep one point of contact for your paralegal or case manager.

Questions firms ask
Before you send the first client.
Do you accept letters of protection (LOP) from personal injury attorneys?+
Yes. Meridian Integrative Wellness treats personal injury clients under a letter of protection at both the Jacksonville (Southside) and Orange Park offices. If your state uses the term medical lien or doctor's lien, it is the same arrangement. Send the LOP with the referral and we schedule the client the same day.
How fast do you turn around medical records and bills?+
Same business day. Send a signed HIPAA authorization and the request by fax (888-502-0725) or through the form on this page; records, itemized CPT-coded billing, and progress notes go back to your office the same day.
Do your doctors provide MMI determinations and permanent impairment ratings?+
Yes. Our chiropractic physicians document maximum medical improvement and provide permanent impairment ratings, along with a narrative report covering mechanism of injury, objective findings, diagnosis, causation, treatment, and prognosis.
Can an attorney or paralegal in another state refer a client to you?+
Yes. We accept referrals from firms in every state and the District of Columbia for clients who were injured in or near Jacksonville — travelers on I-95 and I-10, military families at NAS Jacksonville and Mayport, students, seasonal residents, and people who moved after the crash. Whatever your state calls the arrangement (LOP, lien, med-pay, no-fault), tell us on the form and we will confirm how it works in Florida.
What is Florida's 14-day rule and does it apply to my out-of-state client?+
Florida PIP requires initial medical care within 14 days of a motor-vehicle accident or PIP medical benefits are forfeited. It applies to crashes that happen in Florida regardless of where the client lives. We offer same-day appointments so no referred client misses the window.
Which office should my client use — Jacksonville or Orange Park?+
Jacksonville (9770 Old Baymeadows Rd #139) serves Southside, Baymeadows, Deerwood, Mandarin, the Beaches, and downtown. Orange Park (1532 Kingsley Ave #102) serves Clay County, Fleming Island, Middleburg, Oakleaf, and west Jacksonville. Same doctors' standards, same documentation, same phone number.
Do you pay referral fees or share fees with law firms?+
No. Florida's Patient Brokering Act (§817.505) prohibits paying or receiving anything of value for patient referrals, and Florida Bar Rule 4-7.22 restricts what attorneys may accept. We work with firms because our documentation is good, not because anyone is compensated for sending a client.
What happens after I submit a referral?+
Our office calls the client the same business day to schedule, confirms the appointment back to your office, and sends the initial exam narrative after the first visit. You get a single point of contact for records, status, and scheduling.
Do you treat clients who have no auto insurance or whose PIP is exhausted?+
Yes, under a letter of protection. Tell us the billing arrangement on the referral form so the client is not asked for payment at the front desk.
